Our spines are naturally curved in an S-shape in order to act as spring to distribute the force from standing to facilitate harmonious motion. Scoliosis, or abnormal curvature, can serve to disturb that harmony by creating an imbalance in the spine. This can be due to a variety of factors ranging from neuromuscular disorders to degenerative, or arthritic, changes. This abnormal curvature is not only aesthetically displeasing, but it can also be a source of pain, weakness or even breathing difficulty from inability of the lungs to properly expand.
The goal of scoliosis treatment should be based on an informed discussion between the patient and their surgeon. In growing children, there is a concern that the curve may progress, or worsen, as the child’s skeleton matures. Even children with mostly mature skeletons may be at a greater risk of developing pain and further deformity in adulthood if these curves are not appropriately treated. As such, children with any degree of scoliosis must be carefully monitored and there is a role for bracing to try and prevent worsening. In cases where the progression of a scoliotic curve cannot be controlled with bracing, surgical correction is indicated.
In the treatment of adult scoliosis, the goals of treatment are similar in that harmonious balance should be achieved as possible in order to maximize function and minimize disability.
